Licensure Guide

Walk-Through

1. Bring your completed, signed and notarized application for Licensure by Endorsement
2. Pay the associated fee (if you’re paying cash it must be exact change)
3. Bring a copy of your birth certificate AND driver’s license. OR just your United States passport.

Your application will be reviewed and a temporary license will be issued. You must complete steps 4-6 before the temporary license expires in order to receive your permanent license.

Temporary License

1. Mail in your completed, signed and notarized application for Licensure by Endorsement
2. Pay the associated fees
   1. Make check or money order payable to the Missouri State Board of Nursing.
   2. The fee may be a money order, cashier’s check or personal check.
3. Provide evidence of United States citizenship or lawful presence
   1. Bring a copy of your birth certificate AND driver’s license. OR just your United States passport.
      

   Your application will be reviewed and a temporary license will be issued. You must complete steps 4-6  before the temporary license expires in order to receive your permanent license.

4. Go to the Missouri Automated Criminal History System (MACHS) and register using the 4 digit registration number of 0001. After you register, follow the instructions on their website to complete the fingerprint background check.
5. Verify all your licenses
   1. If the state participates in Nursys, you must complete the Nursys verification process, pay their fee and designate Missouri as the receipt of the verification.
   2. If the state does not participate in Nursys, you must contact their state Board directly for further instructions. You will need to send them a Verification of License form to complete and submit to the Missouri Board of Nursing on your behalf.
6. Official final transcript
   1. Request that your nursing education program submits an official final transcript. The transcript must include your current name, date of completion, degree or diploma awarded, a signature if the director/registrar and the school seal or stamp.
7. Evaluation of credentials (if applicable)
   1. If you were licensed by examination in another state or territory and you received your nursing education outside of the United States, in Puerto Rico or in Canada, you need to have a credentials evaluation service, approved by the board, send our office a course-by-course evaluation report or have the Commission on Graduates of Foreign Nursing Schools (CGFNS) send our office your CGFNS certificate.
   2. A Temporary Permit cannot be issued until the course-by-course report or CGFNS certificate and evidence that you were originally licensed in another state or territory by a recognized examination are received and evaluated.
   3. This evaluation must not be more than one year old.
